Trochlear dysplasia is recognized as a key risk factor for recurrent patellar instability. A systematic review of 17 studies found that it is the most important anatomic risk factor. It is important to recognize dysplasia through lateral X-rays and further understand it using three-dimensional imaging. Flat trochleas do not require deepening trochleoplasty, but convex supratrochlear spurs can lead to patellar instability. Medical imaging, such as MRI, can measure the extent of dysplasia and spur size. Treatment options include deepening trochleoplasty and MPFL reconstruction. The thick shell technique and thin flat technique are two ways to perform trochleoplasty. Immediate motion is important for post-operative recovery.
