Video Summary
A 39-year-old man with a severe, 3 × 3 cm trochlear cartilage lesion underwent staged cartilage implantation using two grafts, along with a tibial tubercle osteotomy to reduce pressure on the damaged area. Fifteen months later, during surgery to remove painful screws, arthroscopy showed substantial cartilage fill and good integration with surrounding cartilage, with little inflammation or delamination. The patient’s functional score had improved markedly, and he had returned to running and basketball. Some untreated cartilage remained irregular, leading the surgeon to conclude that a third graft might have improved the result.
