Video Summary
A 31-year-old former triathlete had persistent patellofemoral pain despite physical therapy, injections, and activity modification. After microfracture of a large trochlear cartilage lesion, he remained symptomatic and imaging showed subchondral marrow edema and an intralesional osteophyte. Dr. Lee chose CartiHeal implantation to address both the damaged subchondral bone and cartilage, placing two plugs recessed beneath the cartilage. At six months, the patient had resumed squatting and most activities, with residual pain during stair descent and eccentric loading. He was rebuilding strength and hoped to return to triathlon training.
