Video Summary
This webinar focused on MACI/Macy cartilage repair of the knee, including indications, surgical planning, patient counseling, outcomes, and rehabilitation. The host reviewed the evolution of the technique from early cell suspension methods to modern collagen-membrane implantation and arthroscopic options for smaller defects. Panelists emphasized that cartilage defects often require individualized treatment based on lesion shape, location, and associated problems such as patellar instability, ACL deficiency, meniscal deficiency, or malalignment.

Several surgical pearls were discussed, including performing a diagnostic arthroscopy before implantation, choosing biopsy sites carefully, and deciding the order of combined procedures like MPFL reconstruction, tibial tubercle osteotomy (TTO), ACL revision, meniscal transplant, and distal femoral osteotomy (DFO). The surgeons stressed that alignment correction is often crucial for success, especially in patellofemoral cases.

Case examples showed good outcomes in complex patients, including athletes and military personnel. One retrospective series of 35 MACI patients reported high satisfaction, strong return-to-sport rates, and relatively few complications, with most patients returning to high-level activity by 9–12 months. Rehabilitation experts highlighted the importance of a team-based approach, patient adherence, gradual progression, and communication with therapists and surgeons. The session closed by encouraging use of support resources, including MACI mentors and credentialed therapists.
