They discuss the risk of failure in arthroscopic stabilization for patients with significant bone defects and the use of a surgical method called arthroscopic remplissage, pioneered by Dr. Wolfe, to fill the Hill-Sachs defect with soft tissue. They conducted a level 1 prospective randomized trial involving 108 patients split into two groups: bank art repair alone or with remplissage. The primary outcome was the WOSI score, and secondary outcomes included repeat dislocations, ASES and SST scores, range of motion, posterior shoulder pain, and Hill-Sachs healing. Results showed no significant differences between the groups, except for a trend towards lower rates of re-dislocation in the remplissage group. Longer-term follow-up is needed to draw concrete conclusions.
