The speaker presents two elbow dislocation cases to highlight management and an underrecognized diagnosis. First, a 29-year-old weightlifter suffered an audible, grossly unstable elbow dislocation with a small coronoid fracture. Instead of external fixation/pins, the surgeon used strong internal suture-anchor “internal brace/X-fix” repairs on both medial and lateral sides, allowing motion starting day two and surprisingly rapid return to light lifting. Second, a 16-year-old with a dislocated, “perched” radial head had an unusual blockage: the superficial head of the brachialis tendon wrapped around the radial neck, preventing reduction. Recognizing and releasing/redirecting it restored reduction and function.
